Phases of Treatment Transition Phase The primary task in the Transition Phase is preparation for discharge. Preparation for discharge requires a thorough understanding of how to develop a Relapse Prevention Plan, how to modify the plan to adjust to new situations, and a willingness to use the principles of relapse prevention at all times in all situations. A comprehensive Relapse Prevention Plan must address all aspects of one’s life. To develop a comprehensive plan, the adolescent must use everything he has learned and achieved in treatment. During this phase, the resident will have increased opportunities to have new experiences and to spend more time off campus in situations that he must face after discharge from New Hope. He will prepare Relapse Prevention Plans for these events and after the event he will analyze how well he followed his plan. He will review this work with his therapist, process group, and parents or treatment ally. These experiences and an overall pattern of stable behavior will demonstrate the degree of readiness for discharge. |
